1、在抗癲癇藥物治療過程中，并不推薦常規監測抗癲癇藥物的血藥濃度。只有當懷疑患者未按醫囑服藥或出現藥物毒性反應、合并使用影響藥物代謝的其他藥物以及存在特殊的臨床情況(如癲癇持續狀體、肝腎疾病、妊娠)等情況時，考慮進行血藥濃度監測。

2、選擇抗癲癇藥物時總的原則：對癲癇發作及癲癇綜合征進行正確分類是合理選藥的基礎。此外還要考慮患者的年齡(兒童、成人、老年人)、性別、伴隨疾病以及抗癲癇藥物潛在的副作用可能對患者未來生活質量的影響等因素。如嬰幼兒患者不會吞服藥片，應用糖漿制劑既有利于患兒服用又方便控制劑量。

3、抗癲癇藥物治療應該盡可能采用單藥治療，直到達到有效或最大耐受量。單藥治療失敗后，可聯合用藥。盡量將作用機制不同、很少或沒有藥物間相互作用的藥物配伍使用。合理配伍用藥應當以臨床效果最好、患者經濟負擔最輕為最終目標。
