哪些外在因素會(huì)誘發(fā)癲癇病

(1)年齡:10歲前發(fā)病的患者占整個(gè)發(fā)病群體的75%，與該時(shí)期腦發(fā)育不成熟，驚厥閾值不穩(wěn)定相關(guān)。嬰兒大腦對(duì)皮質(zhì)下腦干功能控制不完全，出現(xiàn)嬰兒痙孿癥;另一方面，腦的發(fā)育過(guò)程也影響癲癇的表現(xiàn)形式，如兒童失神癲癇，多在六七歲發(fā)病，表現(xiàn)為頻繁失神發(fā)作，青春期后的常轉(zhuǎn)化為全身性強(qiáng)直一陣攣發(fā)作。

(2)內(nèi)分泌:部分女性患者，其癲癇發(fā)作與月經(jīng)周期有關(guān)，在經(jīng)前或經(jīng)期發(fā)作頻繁。實(shí)驗(yàn)證明，癲癇在雌激素水平和黃體酮急降時(shí)最易發(fā)作。少數(shù)病例則分別僅在經(jīng)期內(nèi)或妊娠早期發(fā)作，分別稱為經(jīng)期癲癇和妊娠期癲癇。

(3)睡眠:原發(fā)性全身性強(qiáng)直一陣攣發(fā)作常在晨醒時(shí)發(fā)作;嬰兒痙攣癥也有類似現(xiàn)象;穎葉癲癇在覺醒時(shí)表現(xiàn)為復(fù)雜部分發(fā)作，而在睡眠狀態(tài)不出現(xiàn)強(qiáng)直一陣攣發(fā)作。

(4)兔疫狀態(tài):近來(lái)臨床及實(shí)驗(yàn)發(fā)現(xiàn)，癲癇發(fā)作與患者免疫功能低下有關(guān)。

(5)誘因:發(fā)熱、睡眠不足、疲勞、飲酒、饑餓、精神刺激、情緒波動(dòng)、心理應(yīng)激、突然停用抗癲癇藥物、各種一過(guò)性代謝紊亂和過(guò)敏反應(yīng)等，均可致癲癇發(fā)作。過(guò)多飲水對(duì)于強(qiáng)直一陣孿發(fā)作，過(guò)度換氣對(duì)于失神發(fā)作，閃光對(duì)于肌陣攣發(fā)作，均有誘發(fā)作用。

小兒癲癇的發(fā)病率很高，其中男孩較女孩多，且發(fā)作類型與發(fā)病年齡有一定的關(guān)系。其中嬰兒痙攣癥多在一歲以內(nèi)出現(xiàn)，運(yùn)動(dòng)型發(fā)作癲癇多在6歲以內(nèi)，失神性癲癇發(fā)作多在8歲以內(nèi)，其他各型癲癇的首發(fā)年齡也多在10歲以內(nèi)。小兒癲癇具有隱蔽性，且多呈局灶發(fā)作，年長(zhǎng)兒抽搐明顯且呈全身性。

其次，反復(fù)發(fā)作，表現(xiàn)為刻板性動(dòng)作

癲癇在不同年齡發(fā)作上的表現(xiàn)會(huì)有區(qū)別，主要與中樞神經(jīng)系統(tǒng)的成熟程度有關(guān)系。小兒癲癇多表現(xiàn)為大發(fā)作，多數(shù)患者都會(huì)出現(xiàn)意識(shí)喪失，部分患者還會(huì)伴有眼球運(yùn)動(dòng)的癥狀，且發(fā)作期間主要表現(xiàn)為刻板的反復(fù)性動(dòng)作。
