癲癇婦女在妊娠期間約有1/3患者的發作頻或發作程度增加，約1/4會減少，多數無明顯變化，但是目前尚不能作出預測，很難預測到哪個患者在妊娠期間病情加深。

引起發作增加的原因主要是孕期孕婦體重增加，血容量加大，肝臟腎臟對藥物的代謝和排除，胃腸道對藥物的吸收下降，從而使穩態水平的血藥濃度下降。另一個主要原因是孕婦害怕抗癲癇藥物影響胎兒的正常發育，在妊娠期間自行減少用量或停藥。
