枕葉癲癇患者的主要癥狀有視覺上的癥狀，病人會出現盲點或者偏盲等癥狀，這里面最常見的是閃光或亮點與光幻覺以及視物變形等。當然也有的患者表現為包括頭痛、嘔吐、腹部不適甚至幻視、幻聽等。發作時可見雙眼強直性偏斜或陣攣，眼瞼抽動和閉合，偶有眼陣攣性癲癇持續狀態。眼睛偏斜的方向多數為發作起源的對側，也有少數為同側。

癲癇患者的眼部和非視覺癥狀都是病人經常出現的視力上的異常，都是因為患者的腦部病變引起視力擴散癥狀，癲癇病人在疾病發作時出現EEG枕區棘慢波的變化會影響視力，癲癇病人的顱內放電和影像學都可以診斷。
