通常來說，對于那些臨床表現為頻繁的部分性發作或多種發作類型伴神經系統或精神行為異常的癲癇患兒，治療的預后效果會比較差，這些病例主要包括先天腦發育異常(如半球巨腦癥、皮質局部發育異常等)或圍產期腦損傷伴發的癲癇、嬰兒痙攣癥、腦炎、顏面血管瘤等患兒。這些病例的共同特點是：多種抗癲癇藥物治療后仍頻繁發作，同時出現認知、語言等神經心理功能障礙和身體發育障礙。

如果藥物治療癲癇病效果不是很好的話，這個時候兒童癲癇能不能治愈呢?患者還可以選擇手術的方法，但是手術對患者也有條件限制。例如需經正規服用抗癲癇藥2年時間仍不能控制發作，頻率每月2次以上，嚴重影響日常工作和生活的患者或者癲癇發作已經影響到神經發育的患者可以嘗試手術治療，治療時期一般選在5~10歲之間較為理想，所以說，不是每個人都適合手術方法來治療的。
