懷孕極易導致癲癇復發，懷孕前已患有癲癇，病情又未能完全控制的患者，孕后由于體內水鈉潴留、電解質紊亂、情緒緊張、內分泌和呼吸方式改變等等的變化，都可以使癲癇發作變得頻繁;惡心嘔吐等早孕反應，使吸收抗癲癇藥物受到阻礙，使血漿中的藥物濃度下降，促使癲癇復發。與此相似，抗癲癇藥物在體內的代謝也會因懷孕而發生變化。血漿加大清除力度，血液濃度也會下降。所以癲癇病人在懷孕期間應該增加1/3量的抗癲癇藥，以防止癲癇復發。但由于抗癲癇藥物有造成胎兒畸形的作用，增加藥量應當慎重。

癲癇極易誘發妊娠并發癥。病情沒有完全緩解的癲癇患者出現妊娠并發癥的比正常孕婦高;分娩時引產和使用產鉗的機會多于正常產婦;新生兒的死亡率也高于正常產婦所生嬰幾。
