什么是精神運(yùn)動(dòng)性癲癇

臨床表現(xiàn)可分為：

(1)僅有意識(shí)障礙本型的意識(shí)障礙多在1分鐘以上，而失神發(fā)作多在1分鐘以內(nèi)，腦電圖也沒有失神發(fā)作的3次/秒棘慢波綜合。

(2)識(shí)別性癥狀記憶障礙最常見，有的病人對(duì)本來陌生的人或物產(chǎn)生熟悉的感覺，稱"似曾相識(shí)"感。有的對(duì)熟識(shí)的人或環(huán)境，莫名其妙地產(chǎn)生陌生感覺。有的對(duì)某個(gè)事件產(chǎn)生"似曾經(jīng)歷感"。

(3)情感障礙可產(chǎn)生發(fā)作性的情感異常，如突然感到憂傷、憤怒、恐懼、高興、得意、性興奮、大禍臨頭、末日來臨等。

(4)精神感覺癥狀如聽錯(cuò)覺發(fā)作時(shí)，別人對(duì)自己的談話像是隔了一堵墻。視錯(cuò)覺感到看到的東西像蒙了一層紗。看見地面起伏不平，看到物體像被扭曲了。還有視物變大，視物變小(周圍的人都像是"小人國"的人)等。幻覺也是常見的癥狀，如有一患兒發(fā)作時(shí)表現(xiàn)驚恐，逃跑。發(fā)作后詢問，她說每次發(fā)作時(shí)都看見一位白胡子老爺爺，背一個(gè)包袱，像圣誕老人，在追趕她。幻覺鮮明、生動(dòng)、病人的情緒也受其支配。另外，有的病人對(duì)當(dāng)時(shí)的環(huán)境及人有一種虛幻的感覺，像做夢(mèng)一樣。有一位病人訴說，發(fā)作時(shí)如身臨音樂廳，看到了朋友并聽到演奏。此時(shí)他自己也知道，并非在音樂廳。

(5)精神運(yùn)動(dòng)癥狀以自動(dòng)癥最常見。口咽部不自主的動(dòng)作，如吸吮、咀嚼、吞咽等。有的病人手擦衣服、手舉空中劃圈等。有時(shí)較為復(fù)雜的自動(dòng)癥則表現(xiàn)夢(mèng)游及神游等。如一位病人獨(dú)居，早晨發(fā)現(xiàn)室內(nèi)物品混亂，才知自己昨夜又發(fā)病了。有一病人發(fā)病時(shí)騎摩托車環(huán)城游蕩，并順利返回住所，而自己毫無所知。

(6)復(fù)合型表現(xiàn)為多種復(fù)雜癥狀的綜合。有的突然暴發(fā)沖動(dòng)，甚至產(chǎn)生違法行為，如傷人、毀物、自傷、殺人等。有一農(nóng)村老婦，鄰居出工將小孩托其照看，收工后發(fā)現(xiàn)小孩被砍成幾塊，煮在鍋里。被捕后，審問中她自己也莫名其妙，對(duì)事情一無所知。經(jīng)鑒定，結(jié)合有癲癇史及腦電圖異常，結(jié)論為癲癇性病理激情發(fā)作。精神運(yùn)動(dòng)性發(fā)作腦電圖檢查時(shí)，應(yīng)采用睡眠、藥物誘發(fā)等，蝶骨電極和深部電極可提高陽性率。腦電圖可見一側(cè)或以側(cè)。
