不論任何一種癲癇,在發(fā)作時(shí)均伴有腦部的癇性放電,但目前臨床上所使用的常規(guī)頭皮腦電圖不一定都能記錄到,所以,臨床上診斷時(shí)一定要結(jié)合病史,體檢和其他方面的內(nèi)容進(jìn)行綜合分析,不能因?yàn)槟X電圖呈陰性就否認(rèn)是癲癇,當(dāng)然沒有臨床表現(xiàn)而腦電圖呈陽性者,也是不能確診的。

在診斷時(shí)應(yīng)弄清以下幾個(gè)問題：①發(fā)作性癥狀是否為癲癇;②如果是癲癇,是什么類型的發(fā)作,是否為特殊的癲癇綜合征;③如果是癲癇,是否存在致癇病灶,是否有誘發(fā)因素,誘因是什么。因大多數(shù)患者發(fā)作時(shí)有一定的意識障礙,本人對發(fā)作過程不能表達(dá),又因?yàn)獒t(yī)師很少目睹患者的發(fā)作過程,所以詳確的病史主要靠患者家屬或目睹者陳述出來,密切配合醫(yī)師做正確的診斷,以便及時(shí)得到有效的治療,防止病情加重。
