其中眾多學(xué)者公認(rèn)的對(duì)大多數(shù)繼發(fā)性癲癇病因的明確有重要價(jià)值的方法是CT和MRI，是診斷顱內(nèi)結(jié)構(gòu)性病變(如腦腫瘤、血管畸形、腦囊蟲、異常鈣化、腦發(fā)育畸形等)的重要方法，結(jié)合腦電圖可診斷癲癇并對(duì)致癇灶定位。其中常規(guī)MRI能查出95%腫瘤、錯(cuò)構(gòu)瘤、腦的發(fā)育異常,并能測(cè)定出顳葉內(nèi)側(cè)的海馬硬化。隨著影像學(xué)的不斷發(fā)展，各種先進(jìn)技術(shù)的不斷應(yīng)用和成熟，PET、SPECT、MRS、MEG等也成為對(duì)癲癇診斷的重要工具。PET在癲癇術(shù)前評(píng)估起到很重要的作用，還有可能在特定癲癇人群中發(fā)現(xiàn)新的癲癇病因。

2電生理學(xué)檢查臨床診斷癲癇時(shí)，除了詳細(xì)的了解病史及發(fā)作時(shí)的表現(xiàn)外，首先應(yīng)作的就是顱外腦電圖(EEG)檢查，它對(duì)于癲癇的診斷、癲癇發(fā)作的分類、明確癲癇綜合征尤其是致癇灶的精確定位方面具有重要的意義。如能重復(fù)檢查并使用適當(dāng)?shù)恼T發(fā)試驗(yàn)(過(guò)度換氣，閃光刺激，藥物，睡眠等)以及結(jié)合特殊電極，即使在發(fā)作間期，其陽(yáng)性率可增加到90%左右。近年來(lái)，隨著科技的進(jìn)步，24小時(shí)長(zhǎng)程腦電監(jiān)測(cè)及錄像EEG也在臨床上得到了廣泛的應(yīng)用，更大大的提高了癲癇類型、定位的診斷準(zhǔn)確率。

另外術(shù)前明確致癇灶的方法還有深部腦電圖(DEEG)、皮質(zhì)腦電圖(ECoG)及立體定向腦電圖(SEEG)等，這些方法能準(zhǔn)確的定出致癇灶，但都為創(chuàng)傷性檢查，易出現(xiàn)各種并發(fā)癥，故臨床上不常使用。

而術(shù)中ECoG是目前臨床上常用的癲癇灶定位方法，它對(duì)致癲癇灶的陽(yáng)性發(fā)現(xiàn)率和定位定側(cè)準(zhǔn)確率都明顯優(yōu)于顱外EEG和神經(jīng)影像學(xué)，而且它既能保證有效的切除足夠范圍的致癲癇灶，又能起到最大限度的保留重要的功能神經(jīng)區(qū)，提高了手術(shù)治療癲癇的效果，保證了患者術(shù)后的生活質(zhì)量，是目前眾多學(xué)者公認(rèn)的尋找致癲癇源的主要方法。
