1、癲癇婚育的遺傳因素有多大

通常以為，癲癇有一定的遺傳影響，調(diào)查122對(duì)至少有—個(gè)癲癇患者的雙生子，其中69對(duì)單卵子雙生子中，有44對(duì)兩人都患癲癇，占63%，53對(duì)雙卵雙生中，5對(duì)患癲癇者占9%。經(jīng)過對(duì)癲癇患者雙生子的調(diào)查研究，證實(shí)癲癇婚育是遺傳的;所以，原發(fā)性癲癇病人雖可結(jié)婚，但應(yīng)限定生育。癲癇患者在挑選配偶時(shí)，應(yīng)注意不與家庭中或本人有癲癇和高熱驚厥史的對(duì)象結(jié)婚，不應(yīng)與血緣關(guān)系近的人結(jié)婚。所說原發(fā)性癲癇指的是，起病在25歲下面，發(fā)作形式為大發(fā)作或小發(fā)作，腦圖和神經(jīng)診斷無局限性腦損傷，及其未找到明確的致病因素。

2、抗癲癇藥物對(duì)癲癇孕婦的影響

癲癇患者孕婦，癲癇患者有身孕后，45%發(fā)作次數(shù)變多。這主要由于有身孕后液體潴留，藥品在肝內(nèi)代謝變快，胎兒器官和胎盤容易變多，引致抗癲癇藥品血清濃度下降所形成。發(fā)作次數(shù)與發(fā)作嚴(yán)重水平變多與否，與有身孕前癲癇控制的水平緊密相關(guān)。所以，癲癇患者，必需做好孕前準(zhǔn)備，至少達(dá)到1年上面嚴(yán)格控制。鑒于變多抗癲癇藥品的量，可能對(duì)母親及胎兒產(chǎn)生不良影響，有身孕后要常常檢查癲癇藥的血濃度，若低于有效藥品水平，可合理變多藥量，切不可盲目加量。

3、癲癇婚育應(yīng)注意的問題

(一)病癥獲得較好的控制時(shí)再婚育。在發(fā)作頻繁、病癥較重時(shí)，不應(yīng)思考婚育問題。癲癇發(fā)作會(huì)引致全身缺氧，在進(jìn)一步影響到胎兒缺氧，使胎兒的正常發(fā)育受到影響。此外，在癲癇活躍期，患者使用的抗癲癇藥品有很強(qiáng)的毒副作用，可能引致胎兒發(fā)育不全，也可能引致胎兒變形。

(二)最好不應(yīng)該再找癲癇患者，或者是先天性疾病的患者作配偶。否則，他們的子女受遺傳因素影響較大，癲癇患病率明顯增高。

(三)正在使用抗癲癇藥品的婦女不應(yīng)哺乳。嬰兒對(duì)藥品的排泄速度慢，相對(duì)比較脆弱，吃了含有抗癲癇藥品的乳汁，會(huì)有很大損傷。

(四)不良情緒會(huì)引發(fā)癲癇，當(dāng)患者身心壓力很大時(shí)，對(duì)病癥的穩(wěn)定非常不利。孕期癲癇患者更應(yīng)該注意調(diào)理自身的情緒，當(dāng)遇到困難或疑問時(shí)，趁早獲得醫(yī)生的專業(yè)指導(dǎo)是非常重要的。
