目前認為：大腦皮質、灰質、腦干、小腦和脊髓等的不同平面均可引起肌陣攣，其中以腦干下段的網狀結構，特別是延腦的巨細胞核放電在發病中的意義最大，另外，也可能是繼發于腦部的疾病，如神經系統變性、腦外傷、感染等。
