1、引起羊癲瘋的病因未能發現并消除，如顱內各種性質的占位疾患及腦血管畸形等引起的羊癲瘋發作，若不針對病因進行治療，單用抗羊癲瘋藥物則收效不大。

2、在服抗羊癲瘋藥物的過程中產生了藥物耐受性，如卡馬西平具有誘導自身代謝的作用，連續服藥后可致血漿藥物濃度降低，療效下降，所謂代謝耐受現象，可通過增加劑量來提高血清藥物濃度及療效。而苯二氮卓類抗羊癲瘋藥物連續服用數月后在血清藥物濃度不變的情況下亦出現療效降低，此謂功能性耐受現象，一旦產生，增加劑量無效，需要更換其它藥物。

3、患者不能積極接受治療，不能按照醫囑堅持長期規律服藥，即患者順應性較差。

4、頻繁更換抗羊癲瘋藥物、加藥、或撤藥時又未按照遞增或遞減的規律進行。

5、在抗羊癲瘋藥物治療中，出現藥物副作用，迅速終止該藥，而新換的藥物尚未達到有效血清濃度，致使羊癲瘋發作。
