不同類型癲癇發(fā)作介紹

植物神經(jīng)性癲癇，植物神經(jīng)性癲癇發(fā)作時(shí)，有輕重不等的意識(shí)障礙，發(fā)作后可有嗜睡，發(fā)作持續(xù)數(shù)分鐘至數(shù)小時(shí)，甚至1-2天，發(fā)作頻率不定，可表現(xiàn)為腹痛或嘔吐及頭痛癥狀。部分性癲癇，部分性癲癇為軀體局部肌肉或肢體的抽搐發(fā)作。持續(xù)時(shí)間可能比較長，一般沒有意識(shí)喪失，嚴(yán)重發(fā)作以后抽搐部位的肌肉可有暫時(shí)性麻痹。

全身性發(fā)作，癲癇全身性發(fā)作，也叫癲癇的大發(fā)作，有以下常見的臨床癥狀。全身性發(fā)作患者半數(shù)有先兆，如頭昏、精神錯(cuò)亂、上腹部不適、視聽和嗅覺障礙。發(fā)作時(shí)，有些病人先發(fā)出尖銳叫聲，后既有意識(shí)喪失而跌倒，有全身肌肉強(qiáng)直、呼吸停頓，頭眼可偏向一側(cè)，數(shù)秒鐘后有陣攣性抽搐，抽搐逐漸加重，歷時(shí)數(shù)十秒鐘，陣攣期呼吸恢復(fù)，口吐白沫。部分病人有大小便失禁、抽搐后全身松弛或進(jìn)入昏睡，此后意識(shí)逐漸恢復(fù)。

精神運(yùn)動(dòng)性發(fā)作，精神感覺性、精神運(yùn)動(dòng)性及混合性發(fā)作。多有不同程度的意識(shí)障礙及明顯的思維、知覺、情感和精神運(yùn)動(dòng)障礙。可有神游癥、夜游癥等自動(dòng)癥表現(xiàn)。有時(shí)在幻覺、妄想的支配下可發(fā)生傷人、自傷等暴力行為。失神發(fā)作，突發(fā)性精神活動(dòng)中斷，意識(shí)喪失、可伴肌陣攣或自動(dòng)癥。一次發(fā)作數(shù)秒至十余秒。腦電圖出現(xiàn)3次/秒棘慢或尖慢波。

大發(fā)作,病人突發(fā)意識(shí)喪失、跌倒有時(shí)大叫一聲、呼吸暫停、口吐白沫、全身強(qiáng)直性抽搐、唇舌咬破，大小便失禁。5-10分鐘恢復(fù)。小發(fā)作,癲癇病醫(yī)院專家說，失神小發(fā)作,突然兩眼凝視或上翻，楞神，活動(dòng)、語言中斷，持物掉地，呼之不應(yīng)，約數(shù)十秒鐘。肌陣攣小發(fā)作,面部、上肢、頸部、軀干發(fā)生短促(1—2秒)的肌陣攣。

局限性發(fā)作,一側(cè)口角、眼瞼、手指、足趾或一側(cè)面部及肢體末端短陣性抽搐或麻木刺痛。抽搐有時(shí)可由手指至上肢擴(kuò)展到對(duì)側(cè)。精神運(yùn)動(dòng)性發(fā)作,類似失神小發(fā)作，但持續(xù)1分鐘以上。或出現(xiàn)多種幻覺、錯(cuò)覺、無意識(shí)的動(dòng)作，如吸吮、咀嚼、咂嘴、脫衣、解紐扣等。

癲癇發(fā)作時(shí)的表現(xiàn)形式多種多樣，但都具有突發(fā)突止和周期性發(fā)作的特點(diǎn)。小兒常見的有大發(fā)作、失神小發(fā)作及小兒良性癲癇。大發(fā)作時(shí)患兒突然神志喪失，呼吸暫停，面色青紫，瞳孔散大，四肢強(qiáng)直，雙手握拳，然后轉(zhuǎn)入陣發(fā)性抽搐，口吐白沫，發(fā)作一般持續(xù)1～5分鐘。失神小發(fā)作的小兒表現(xiàn)為突然意識(shí)喪失，活動(dòng)中斷，兩眼凝視或上翻，但不跌倒，不抽搐，歷時(shí)1～10秒鐘，發(fā)作后意識(shí)很快恢復(fù)。小兒良性癲癇發(fā)作時(shí)多為一側(cè)面、唇、舌的抽動(dòng)，可伴有該部的感覺異常，不能說話，流涎，一般神志清楚，夜間發(fā)作多，預(yù)后比較好。
