一我國癲癇發作分類法(草案)

(一)部分性發作(局限性、局灶性)

1、單純部分性發作，無意識障礙

(1)運動性(局限性、局灶性)

(2)感覺性(軀體性、特殊感覺性)

(3)自主神經性

(4)精神性(見復雜部分性發作)

2、復雜部分性發作(精神運動性發作或顳葉癲癇)，伴有意識障礙

(1)僅有意識障礙

(2)精神癥狀(感知、情感、記憶、錯覺、幻覺等)

(3)自動性

(二)全身性發作(普遍性)：非局限開始

1、全身強直一陣攣發作(大發作)

2、失神發作(小發作)典型或不典型

3、其它肌陣攣發作、陣攣發作、強直發作、失張力發作

(三)不能分類，因資料不足或不能歸入上述各類的發作

附：

1、癲癇持續狀態

(1)全身強直陣攣發作持續狀態

(2)失神發作持續狀態

(3)復雜部分性發作持續狀態

(4)部分性癲癇持續狀態

2、在某些特定情況下發生的發作

(1)反射性發作

(2)各種誘發因素引起的發作(如飲酒、疲勞、情緒等)

(3)周期性發作(如月經、覺醒睡眠周期等)

二、小兒癲癇發作分類

(一)部分性(限局性、局灶性)發作

1.簡單部分性發作

(1)限局性運動性發作(包括局限性軀體運動性發作、杰克森發作、轉側性發作等)。

(2)限局性感覺性發作(包括軀體感覺性發作和特殊感覺性(如視、聽、嗅、味、眩暈等)發作。

(3)限局性植物神經性發作

(4)限局性精神癥狀性發作(包括失語、記憶障礙、認知障礙、錯覺、幻覺及其它意識障礙之外的高級腦功能紊亂)。

2.復雜部分性發作

開始為簡單部分性發作，繼之(或開始即)出現障礙(程度不等);伴有自動癥。

3.部分性發作演變為全身性發作

(二)全身性(廣泛性、彌漫性)發作

1、失神發作

2、肌陣攣性發作

3、陣攣性發作

4、強直性發作

5、強直一陣攣發作

6、失張力性發作

(三)其它分類不明的備種發作

三、癲癇分類建議

(1995年全國第七屆小兒神經學術會議)

(一)表現為部分性(限局性)發作的癲癇

1、原發性(特發性)

(1)具有中央一顳部棘波的小兒良性癲癇

(2)具有枕區放電的小兒癲病

2、續發性(癥狀性)或隱源性

(1)小兒慢性進行性部分性連續性癲癇

(2)額、顳、頂或枕葉癲癇

(二)表現為全身性發作的癲癇

1、原發性(特發性)

(1)良性家族性新生兒驚厥

(2)良性新生兒驚厥

(3)良性嬰兒肌陣攣性癲癇

(4)小兒失神癲癇

(5)少年失神癲癇

(6)少年肌陣攣癲癇

(7)覺醒時強直一陣攣大發作性癲癇

2、繼發作(癥狀性)或隱源性

(1)小嬰兒癲癇性腦病伴爆發抑制(大田原綜合征)

(2)嬰兒痙攣

(3)Lennox-Gastaut綜合征

(4)肌陣攣起立不能性癲癇

(三)尚不能確定是部分性或全身性發作的癲癇

1、嬰兒期嚴重肌陣攣性癲癇

2、發生于慢波睡眠時有持續性棘慢波的癲癇

3、獲得性失語性癲癇

(四)各種誘發因素促發的癲癇及特殊綜合征

1、熱性驚厥

2、反射性癲癇
