1、如果繼續發作，藥物可增加至耐受劑量;發作仍不能控制時，該藥應減量，轉換為不同藥理機制的另1種一線藥物的平均劑量。當第2種單藥治療反應良好，可緩慢撤除原藥。當藥物極大劑量仍不能控制發作，應警惕抗癲癇藥物引起的癇性發作或藥物選擇不當。

2、當選擇第一線抗癲癇藥物單藥治療時，初始從小劑量并逐漸滴定至低的維持劑量。不屬緊急治療不采取藥物負荷方式。

3、在添加治療時，應緩慢將已有不良反應的第1種藥物減至中等劑量，之后再添加不同藥理機制的另1種藥物。兩種藥物的聯合應用療效似乎優于先后替換兩種藥物的單藥治療。如果患者兩種藥物的聯合應用在3月內仍未見效，應緩慢轉變為第2種藥物的單藥治療，并開始添加新選擇的抗癲癇藥物。

4、對癇性發作應明確是事件相關性發作，還是內科疾病引起的抽搐，或是很少發作僅表現為輕度抽搐的良性癲癇。針對內科疾病引起的癇性發作，應針對原發病進行治療，僅僅是在患者反復抽搐、誘發因素不能糾正時才需要抗癲癇藥物長期治療。失神性癲癇狀態或非驚厥性簡單部分性癲癇狀態，沒有腦損傷的肯定證據，且預后較好，一般不需要過激的抗癲癇藥物治療。非癇性發作的誤診常引起不恰當的抗癲癇藥物治療。
