老年人癲癇患者對藥物的不良反應的發生率更高，尤其是卡馬西平，更容易加重共濟失調，因藥物的不良反應更容易中斷抗癲癇藥物的連續服用。由于老人可能合并其它的內臟疾病，需要同時服用其它藥物，所以在癲癇藥物選擇上盡可能避免藥物間的相互作用。可選擇新一代抗癲癇藥物，盡可能選擇單一抗癲癇藥物。

老年人癲癇患者抗癲癇藥物的敏感性增加、腎小球的濾過下降，藥物在體內的存留時間更長，所以采用低于平均劑量也會收到很好的療效。同時需要選擇盡可能不通過腎臟濾過的藥物。而且由于體內脂肪含量、白蛋白和細胞色素P450的變化，奧卡西平在老年組更易發生低鈉血癥。
