癲癇病怎么進(jìn)行治療

患者：患者的小腦扁桃體下疝及脊髓空洞癥是出血形成的呢，還是其它原因形成的。是否還會發(fā)展?

癲癇專家：這種病變大多見于先天性畸型，是發(fā)育異常造成的，一般不會進(jìn)一步發(fā)展，如果有進(jìn)展可以做后顱凹切除術(shù)。

患者：患兒個人史中提到曾頻繁出現(xiàn)呼吸、心跳停止，不知是否肯定?

癲癇專家：患兒當(dāng)時病情較重曾出現(xiàn)持續(xù)昏迷幾天，有多次呼吸停止，心跳是否完全停止可能描述的不準(zhǔn)確。

專家：患者是否做了腦電圖?藥物治療劑量如何?

患者：由于不能做腦電圖，所以沒做。藥物劑量是：丙戊酸納40mg/日，魯米那30mg/日，但患者沒有堅(jiān)持規(guī)律用藥。

癲癇專家說：對于這個病人的診斷我同意羅教授的意見。致于發(fā)病的原因，我認(rèn)為與患兒出生后期8天出現(xiàn)的顱內(nèi)出血、缺氧有明顯的關(guān)系，造成繼發(fā)性癲癇，現(xiàn)在因左側(cè)大腦半球萎縮，使癲癇進(jìn)一步加重，與小腦關(guān)系不大。

患者問：小腦扁桃體疝的出現(xiàn)是先天性畸型造成還是新生兒出血時造成，因?yàn)榛純寒?dāng)時顱內(nèi)出血很多。

專家答：兩種原因都有可以，臨床上都能見到。但患者左大腦半球完全萎縮與7年前的外傷無關(guān)系，因?yàn)楫?dāng)時沒出現(xiàn)昏迷，不會造成大腦萎縮，而與新生兒期顱內(nèi)出血有關(guān)。

患者問：如果用藥，是二種藥共用，還是一種藥?

癲癇專家說：既然患者沒有規(guī)律用過藥，我認(rèn)為應(yīng)讓患者在家長的協(xié)助下系統(tǒng)用藥，觀察三個月，如果藥物不能控制再考慮手術(shù)，這樣給患者一個避免手術(shù)的機(jī)會。原則上是單一用藥，因?yàn)橥瑫r用二種藥可引導(dǎo)肝內(nèi)產(chǎn)生相應(yīng)的抗藥酶，破壞藥物的作用。如果這個患者單一用藥不理想，可以以丙戊酸鈉為主，配合小劑量的魯米那。另外建議你們給患者作腦電圖檢查，這對于判斷癲癇類型和指導(dǎo)用藥有好處。

癲癇專家說：一般說，藥物治療二年效果不明顯可考慮手術(shù)。
