外傷性羊角風(fēng)均有頭部外傷史可查，不論是閉合性或開放性顱腦損傷，傷后不同時期出現(xiàn)的不同類型羊角風(fēng)發(fā)作，特別是腦組織損傷部位與癇灶相符合的局部性發(fā)作而傷前無羊角風(fēng)病史的患者，不難確診。

外傷性羊角風(fēng)的診斷，除臨床表現(xiàn)及其特點之外，尚須依靠腦電圖檢查。源于大腦皮質(zhì)的羊角風(fēng)波常為高波幅的尖波、棘波、尖慢波或棘慢波綜合，位相一般為陰性;病灶深在者，其波形多為尖波或尖慢波綜合，波幅較低，位相有時陰性，有時陽性。羊角風(fēng)灶的定位，除根據(jù)波形、波幅及位相之外，尚應(yīng)注意癇波出現(xiàn)的同步性。兩個以上同步的羊角風(fēng)波，有時來自同一病灶，呈現(xiàn)雙側(cè)同步的陣發(fā)性慢波，一般認為中央系統(tǒng)發(fā)作，或陳舊性羊角風(fēng)。

此外，腦CT或MRI掃描亦有助于了解病灶的部位和性質(zhì)，通常可見局限性或彌漫性腦萎縮、腦膠質(zhì)增生或囊性病變、腦穿通畸形、蛛網(wǎng)膜囊腫、腦池擴大、腦室受牽扯、骨折片陷入、血腫、膿腫及異物等。
